Draft Registration Form
From KM4DevWiki
Second draft registration form:
The KM4Dev 2008 meeting will take place at the Pousada de Juventude Hostel in Almada, Portugal (just outside Lisbon) from June 18 - 21! The closest airport is Lisbon and there is public transportation available from the airport to the Hostel (approximately 45 minutes).
This year, we will start our gathering on June 18th with a day that combines several short introductory sessions on different aspects of KM4Dev along with some networking time for the KM4Dev community to get to know other communities with complementary interests. This is a great opportunity for new members to learn more about the community and its members.
From June 19-21, we will have interactive sessions on a variety of topics chosen by KM4Dev participants using the Open Space method. You'll find more details on the program as it develops here (link).
Which days to attend? Be sure to attend June 18th if you are relatively new to KM4dev or looking for networking opportunities. We will start early on June 19th, so make sure to arrive the day before, and we will finish by 13:00 on the afternoon of the 21st. It is a lovely area, so you may want to arrive early or stay another day and go to the beach or see Lisbon.
If you plan to attend, please register by April 30th, answering the following questions. Registration is is limited and will be accepted on a first-come, first-served basis.
1) Please enter the following contact information
a) Name(s), SURNAME (example: Jopie Klaas VAN DALEN)
a2) I am male / female (for accomodation purposes)
b) organisation (optional)
c) mailing address, including country
d) email
e) tel, including country code(optional)
f) cellphone, including country code (optional)
g) skype name (optional)
h) fax (optional)
i) your website (optional)
2) Costs and Accomodation:
There are no fees to attend the meeting, however there are costs associated with travel and accommodation, which most participants are expected to fund from their own or their institution's resources and which must be paid upon arrival at the event.
Note: We have VERY LIMITED funding to support 5-7 colleagues from the south. If you would like to be considered for this, please continue to complete the questions in this registration form and you will be prompted to fill out several funding support questions at the end.
The costs vary by type of accommodation and are listed below. All meals will be paid by individuals on site (Breakfast is included in the accomodation price. Lunch and dinner served at the Hostel are 3 course meals and cost 6 Euros).
To make the event affordable, we have chosen to book the Pousada de Juventude Hostel outside of Lisbon, Portugal. This means that in principle all the rooms are shared accommodation in either double rooms (2 beds including bathroom) or in dorm rooms with a shared bathroom outside the room.
We hope to be able to accommodate all participants in rooms with no more than 2 people per room, but there is a chance (if a lot of people attend) that we will need to have up to 3 people in some rooms.
If you want a single room, you will be accomodated at a nearby hotel for a higher cost. You will be responsible for your transportation to and from the meeting venue. (We have a block reservation at the nearby Hotel Costa de Caparica, within 10 minutes of the Hostel by taxi.)
Keeping these things in mind, accommodation will be available in the following three categories.
Please rank in order of preference:
a) I would like to book a double room, sharing room and bathroom with one other person (same sex. Cost - EUR50 per night)
b) I am willing to share a room with up to two other people, with a shared bathroom outside the room (same sex. Cost - EUR25 per night)
c) I would prefer a single room. I will book my own room in Hotel Costa de Caparica. I realize I will be responsible for booking, payment and transportation (10 mins by taxi) to the Almada Hostel. I will pay to the Hotel directly. (Cost - aprox 70 Eur per night if you book quickly and mention "KM4dev". Note that this rate can only be guaranteed until April 10th. After that rates go up).
In the unlikely case that we have rooms to spare, we will try to accommodate singles on site within the Hostel. Please tick here << >> if you would like to be in a single within the Hostel in case this is made possible.)
d) I will participate and may take meals at the Hostel, but do not need any accomodation. (choose this if you live nearby or prefer to make your own, fully independent, arrangements.)
If you plan to bring someone along (family member, friend) who will not be participating in the conference, but who does need accomodation please indicate and specify below:
3) The organizing committee will group people in rooms in the ways they deem best, on the basis of their sex, order of registration and arrival. Of course, your preferences also count. If there is anyone you know will also attend, and who you would like to room with, indicate below. If you do not have a special request, leave blank.
I would like to be placed in a room with __________
4) My arrival date is (no earlier than 17th June):
5) My departure date is (no later than 21st of June):
(arriving before, or staying on after these dates might very well be possible in the Almada Youth Hostel, but needs your own booking)
6) Meals and dietary requirements All meals will be paid by individuals on site. Breakfast is included in the accomodation price. Lunch and dinner served at the Hostel are 3 course meals and cost 6 Euros. There is also a small café within the Hostel.
We will do our best to cater for any dietary requirements. Please indicate a) vegetarian b) halal c) other ......
You are asked to pay upon arrival at the event. Helvetas will administer the accommodation payment process and will issue an invoice to you based on the accommodation category you choose times the number of nights you will stay (e.g., If you are willing to share a room with up to two other people and you are arriving on 17 June and leaving 21 June, you will be charged EUR25 times 4 nights = EUR100).
7) Will you be:
a)self-funded b)requesting funding support
If respondents choose a), they will be directed to the 'thank you' page. If they choose b), they will be directed to the following funding support questions
8) As noted, available funding to support participation in the meeting is very limited and we expect to be able to fund only 5-7 people. We have created a set of criteria based on donor funding requirements. These criteria are:
i) Must be based in the South ii) Must be with an organisation that is based in the South (e.g., not a southern-based office of an international organisation or an organisation with HQ in the North), but could be any of CSO, private sector, government, academic, consultant with registered own business iii) Must have a demonstrable link with the KM4Dev community or a demonstrable KM interest iv) Must be prepared to provide a tangible input to the workshop (e.g., facilitation of a session, short blog posting, sharing of an experience, methodology, etc.).
If you fit these criteria, what level of funding would you like to apply for?
Note: if some participants can attend without full funding, it will allow us to sponsor more people. a) Full funding (visa, travel, accommodation and meals) b) Airfare only c) Accommodation and meals only
9) Please describe your link with the KM4Dev community or with KM more generally
10) Please let us know why you are interested in participating in the workshop.
Both sets of respondents will be taken to the following page when they have completed their questions:
Thank you for registering for the KM4Dev 2008 workshop! We will contact all registrants once the organizing committee has reviewed all applications and accommodation requests. You can expect us to contact you on or before 09 May.
